Hide 'States' list unless USA/Canada Selected V2013

Hi,

I am running Version 12.01 NBLA.

I am trying to apply the code from this link
http://community.sellerdeck.com/showthread.php?t=45511

But the code doesn't seem to work in the new version and the state list is still displayed for all countries.
In fact the list is permanently visible as per screenshot.

Does anyone know of a way to hide States' list unless USA/Canada is selected in V2013
