custom form in a product page

I would like to add a new product - which is actually a training course. I would need to capture customer details prior to the checkout process. Is this possible?
I've seen on Norm's site the 'Multi Other Info' plugin, but I was hoping for a simplier option. Is there a way to create an html form in Dreamweaver and add the code to Actinic.

I've just noticed in the AUG that it's possible to add multile OtherInfo fields into the checkout. Could this be allpied to the product page.
