button's don't work - help needed, please!

Hello,

We offer samples on our website, but suddenly none of the buttons work when a user clicks on them to order a sample.

Here is an example:

Does anyone have any idea what I need to do? The problem is universal across the site and I'm not aware of any settings having been changed to cause the problem.

Any suggestions gratefully received.

Check your layout. The end form tag comes at the start of the form tag, not at the end of the layout meaning options are outside of the form.
