Adding fields to contact form

I have followed the instructions in the SellerDeck v12 advanced user guide to add fields to the contact form. This has worked ok but I have a couple of questions if anyone can please help:

1. If there are any errors on the page, the error message is displayed (correctly), but the values of the new filled in fields have been blanked out. How do I redisplay the entries made?

2. As well as text fields, I wish to add radio buttons and muti-choice entry fields. Can you please point me in the right direction to achieve this.

Take a look at Machform instead, that will do all that you are asking and more and can be easily used in a fragment.
