Create subscribe field in Actinic business V10

I want to add a 'subscribe to receive our catalogue' field on my website homepage (homepage only - not throughout the entire site).

I'd thought of using a fragment, but not sure how to capture the email address - a simple email form where the email address gets sent to the website would be sufficient.

Any ideas how I can do this?

Create a form using machform and embed it into a fragment on just the required page is an option.
