I am setting up a shop to sell tools to fellow bikers. I want to offer discount to members of owners clubs, IAM etc.

I want to create a web form using "free contact form lite" or something similar, as I did here http://www.raj-bristol.co.uk/booking.html for one of my design clients.
I want members of clubs to fill in a form containing all the info that is required to set up a 'customer account' in actinic, but to include stuff like the name of the club they're a member of, together with their membership number. Whilst it would probably be too much hassle to actually check the numbers, at least it would make the average free loader think.
I have never managed to get on very well with formmail, and invariably had to get the techies at my hosting company to get it working for me. I want to stick the form in a widget as a page.
I'm guessing that the best way is to get it working in dreamweaver and then copy the code in with the obligatory !!<< >>!! ?
Has anyone found a way of setting up a customised form in Actinic V10 which is fairly straight forward? I'm not great with code - I'm OK with run of the mill html, but scripts etc make my head spin
