Permutations Restricted to 4 and Disappeared

I added 4 permutations and all was well. Then my customer wanted a fifth one. When I tried to add it, it refused and gave me the message:

"All possible permutations are done. Cannot add another one."

I've been using Actinic since version 7 and have never experienced this. Then all the permutations disappeared, but were still visible in the Content Tree! Needless to say, they were not visible on the website. I have had to add them again in the permutations section, but again, I can only add four.

The help section doesn't mention a limit and there appears to be nothing else on the forum. Can anyone help?

IIRC you need to add another choice first.

Could you state here the exact steps that led to the message, I have tried playing around with permutations my end, but no error comes up. I guess I might be missing something.
