Amending 'Contact Us' form

I know it's somewhere here! There was a thread about amending the standard form so it could become a subscription form and Chris Dicken produced a new formmail script to run it - have searched for ages but can't find it again... Please could someone point me in the right direction as want to use it in order to create a 'join our maillist' form - many thanks.

Many thanks Chris - I've managed to get so far with this but hit what I think is probably a basic problem.

Have got the form as I want but when I upload to my server the form in my browser shows "NETQUOTEVAR:NAMEVALUE" etc in the actual field rather than being blank. How can I stop this from happening? In the code it's showing as:

value="<value="Actinic:Variable Name="MailFormNameValue"/>

and I haven't been able edit the variable.

make sure your link to the contact us page ends in ?ACTION=SHOWFORM. You can do this by using the <actinic:variable name="SendMailPageLink" /> variable.
