creating a form for customers to fill in

I would like to create a form where I can ask customers questions and they can enter an answer and send it to me, is there any way I can do this?

Like a question and answer form, anyone know?

There is a built-in Contact us form which can be modified.

Modification details can be seen in the Advanced User guide page 73.
