Additional checkout form

Hi,

I am trying to set up an additional selection option at the checkout stage.
The complication is that it is based on two selections which are related (array). The purchaser selects a charity to donate to (10 categories, with 10 charities per selection).

I have some javascript that provides the selection but this needs to be in a seperate form (can't nest forms).

Is it possible to create addtional checkout page to enable this selection?
Or any other possible solutions / suggestions ?

Checkout drop downs

Thanks for the welcome Bruce.

Managed to get javascript to work and with some technical help have the drop downs selectable in the checkout page and pass through to the order.

Would it be useful if the solution were posted here?

Absolutely, there is surely going to be someone else looking for that information at some point of time

The solution was to use the 'HowFound' and 'WhyBuy' options to get the selected options carried over, but a detailed solution will be great.
