Store Top Page Problem

I'm having problems whilst testing a new site in V850 - the Store Top page (acatalog/index.html) does not show correctly at all. The design is a very simple external design from DW. In preview and Off-Line Preview the page shows correctly however when uploaded in Test Mode it looks very odd.

I've licenced a new site and redone everything but still have the same problem. I think it must be something related to the external layout but I can't find what it is.

Can anyone offer any ideas please?

Great thanks to Macolm who found the problem.
You cannot use index.html as the external layout file name.

Simple when you think of it.
