How to Collect Customer Feedback

Hi

I am asking what solutions to collecting feedback from customers the community have found.

We wish set up a system so that we can collect feedback from customers about the products and service etc.

I am thinking trying finding a way to send them a email automaticaly a week or so after i have sent the goods ask ing a few simple questions and getting them to mail it back to us, then we can format it all onto a brochure page once a week or so.

Has anyone found a solution to make this all work?

I think it was Smart Decision who had a customer feedback tool for v8 (and a star rating facility) but have not seen this in a while or in v8 guise.
