Rating/Comments Script

Hi,

I have found loads of scripts in php/sql etc for rating my products but they either do too much or too little.

Can anyone suggest anything? I need the following:

1. rating (5 star etc)
2. comment if they want
3. ip/cookie/capcha
4. by product id
5. comment approval is an added bonus
