Items falling off shopping cart

There are few items on my website, that fall off the shopping cart when added

This seems to happen when i try to add the 4th item on to a perticular set of items.

for example i have 3 items in the basket.

i add a fourth,
it removed the 3rd item and puts the 4th in it place


But like i said only happens with a few items

Duplicate product codes ?

Only a suggestion. you could list the pages and items that cause this to happen so people could take a closer look and try to work it out.
