Customer Feedback Displayed on Website

Hi,
I am interested in having a page on my site where customers can leave feedback for their order. Something similar to this http://www.airsportdirect.com/acatalog/Feedback.html

Any advice is much appreciated.

I think that site is just using a mail form to collect comments and then the site owner manually places them in fragments.

Another way of doing it and getting the product details loaded into an email for you is http://www.the-old-smokehouse.co.uk/...od_Smoker.html. Then as Duncan says you just add the comments to a fragment
