Own contact form seperate to the form on the Contact Us page

Hi,

I need to add a contact form on a fragment within different sections asking questions about different products.
I do not want to use the standard contact us page, i need it to be added to the fragment itself.

Does anyone know how to get it to work correctly within an actinic template?
- The page i want to add it to is as follows: (the form does not work at the moment)

get the form working outside of actinic - then place it in the fragment.

There's nothing special required to get a form to work
