Reatining Previous comments on new orders

Hi,
it seems that when a customer puts a comment in their order, this comment is saved and repeated on any further orders they place unless they specifically go in and write a new one.

We had a problem where a customer ordered some extra goods within the comments section and we despatched them, about 6 months later she orders again, we see the repeated message and send out the same request. Obviously had to then sort out and refund.

Is there any way to stop this happening?
