Card details appearing instead of name

I very occasionally get customer's card numbers appearing instead of their names. I had 2 this weekend. I use Protx. Has anyone else had this? Is it Protx, Actinic or customers?

this is a problem between the customers chair and their keyboard. Very little can be done to stop them mindlessly typing in the wrong things. This has however, been discussed recently. Very interesting problem.
