Creating a Survey?

I am trying to create a survey on my Actinic site has anyone any ideas of how I can do this?

The problem I have is the submit button as I am not able to receive the feedback.

I have been able to do this on non-actinic sites but the Perl script that runs the form seems to need differed permissions for the cgi-bin from what actinic has set.

Any help would be great.
