Removing the Delivery country stage of checkout

I have simplified our shipping costs so that we only have one zone (worldwide) and shipping is £3.50 then free on orders over £30.00.

This makes the first checkout screen for "Select delivery country / state" redundant. Does anyone know how to remove this screen completely from the checkout process and shift the checkbox for selecting a separate delivery address onto the "second" checkout screen - Address entry?

No problem.

Just set both 'Business Settings | Shipping and Handling' and 'Business Settings | Tax' to 'Simple'. This should remove the country drop-downs from the first page of the checkout.
