how to get rid of this

This file was deleted some time ago but i still get asked the above searched for the xls file but it is not on the local machine anymore.

how can i remove this from where ever it is?

Is it listed as an 'additional file' or included in a template/customvar somewhere?

thanks for the prompt responce Mike,
Not in the additional files.
Need to check with my partner about the template, I had a look but it's hard to say coz i didn't add it myself, ill let you know
