The Mystery is Solved!
In sorting out the issue of the "Close Window" not showing, I realised that a different template was being specified for the Extended Window layout. It was on "Use Parent" which I wrongly assumed was act_extendedinfo.html
It looks like my original code is O.K. It just wasn't included in the template that was chosen!
