File Upload CGI Script

I am using a neat file upload cgi script called File Manager script from cgi-world.com
I used this to tailor a form which uploads files:
see www.homefrontexperience.co.uk and click submit online link

The form works as it should outside of actinic but when I use it within actinic using INCLUDE within the template:
ie INCLUDE Home_front_Experience_a.html

I now see the variables that are used within the form ie items with $ around them and the form does not upload the files if you type the information in.
What needs to be changed and where so as the variables do not show and the functionality works?

I am not aware of the script but it may be worth checking that any <form> tags are not being broken along the way by inserting your new code
