A customer has managed to order 20 boxes of an item which we have had marked 'hidden' on the website for a while. Could someone have a cached page and order from it even though the item is hidden? Any ideas how else this could happen?

Are you linking to that hidden product through a component? Even if a product is hidden you can still buy it if you are linking to it with a component.
It is true that caching may be a problem, if you definately do not want people to buy that product, it would be better to untick 'Can be Ordered Online' under the 'General tab' of the product.
