all my orders are coming in doubled up

when a customer makes an order they get it twice with 2 seperate order numbers, i get 2 orders recived, paypal only recive and accept 1 payment.
when view orders both are identical....?

I get the same issue very very seldomly (twice in past 12 months) - I have narrowed this down to people with slow connections hitting the final "next" button twice (though that should not work) and people not realising they have actually bought and going back and doing it again if the cart is still showing contents.
