Contact Form

Can someone tell me how to implement the contact form (Act_ContactUs.html)?? I'm sure I must be missing something. I can't find any menu options (v7 trial version) to 'switch it on' and I've tried using NETQUOTEVAR:INCLUDE Act_ContactUs.html but to no avail.
