'Email a friend' pop up

Has anyone done an 'Email a Friend' pop up that takes your name and email address and the friend's name and email address with a submit button, etc? rather than the 'open outlook' version.

I found this before. This is what I am currently using, it is the 'open outlook to send email' version. I am looking for a pop up 'form' version.
