Text input line in product description

I am offering a embroidery service and need the customer to be able to insert up to two lines of text.

I would like this to be part of the product page

I also need this to show on either the packing list or invoice so that we can relate to it

any idea's

have you looked at the other info prompt on the layout tab in product dialog box?

Looked at it Jo

but all it seems to do is open a window with the text i enter in the text box. I thought there was a text entry line for the customer, but when i upload all I get is my text writting.

also had a look at advanced manual which mentions design text 2161 but my site says thats not available
