My Contact US email is showing pre-filled informtion in the name and email address form

Any idea where it is picking this up from as I can't find where this text information is located or what is causing it to be picked up.

It's empty when viewed here.

Perhaps you have something like the Google Toolbar on your system and it's been set to pre-fill form fields with known names (like firstname, surname, etc). These fields usually show in a different colour.
