Urgent : Actinic Orders Lost

Hi,

A client is having a problem where most orders are being received ok, however sometimes a customer goes through the checkout, the money is taken in secpay, but the order is never received??

Anyone have any ideas where to start looking for the problem here? Any help greatly appreciated..
