Extra Fields In Act_Order01

Hi,

I currently have all the normal fields and these are fine but i need to add around 5 fields 2 text fields 2 drop down boxes and also a textbox.

How do i go about doing this? Do i simply add these manually in the Act_Order01.html or as i would imagine it is a little bit harder than that.
