Opt In Email + Where did you find us?

Does anyone know how to create a positive opt in for customers to receive email newsletters etc perhaps by changing one of the existing options such as coupon codes?

Also, is it possible to change the 'Where did you find us?' so it has a drop down box with options?

Take a look under Design | Text > Phase = 0 id=15 ... this can be modified to suit your needs
