html form input on brochure pages

I want to add a html type input form onto a brochure fragment page. Has anybody got a smart way of doing this? I know how to add pages of html in the shop using properties but these don't exist in brocure pages..

Forgot to say, as part of the form which is a questionaire, there are pop up thumbnails!

Just use HTML and put !!< >!! around the code.

yes I have done this with most pages but the pop ups disappear...
