Select Invoice Location

In the checkout page, there are 2 places shown where the customer selectes the "State". How can I remove the 2nd one?
I am in the USA and only sell in USA.

The second instance is related to the shipping address. There is usually a line stating "Select Shipping Destination" between the two. (Not sure why that is missing unless you have modified the template?)

If you only ever want to ship to the invoice address and therefore do not want the shipping address option to appear, the way to do this is described in the AdvancedGuide under "Only Allowing One Address Per Order" (about page 51).
