Sounds strange to me.

Does the payment have a valid order number associated with and is that order number missing from your list of orders? (you might need to place a test order yourself to generate a subsequent order number)

What version of the software is this happening with?

This may happen if you or your ISP have accidentally deleted some *.ord files from the web site. This might effectively happen by the ISP restoring the site from a backup following some system problem.

I forgot that I am running 2 sites, the mystery orders were from the other site, but without the "new order arrived at your site" email.
