I have a client who supply animal medicines via their website. They are currently using Catalog V7. DEFRA have now told sites like theirs that they are now required to gather extra information from the purchasers of certain products. Nothing like making life difficult for the purchaser of these goods! The following would be a typical product and the required fields that they would need:

Download Animal Medicine Datasheet (I would make this a downloadable PDF)
1. Are you the registered keeper/owner of the animal(s) to be treated?
YES/NO
(If no, we need to have the name and address of the registered keeper)
2. What is the species of the animal being treated? Please Select (Drop Down
Box to show Sheep, Cattle, Horses, Goats, Cats, Dogs, Poultry)
3. Flock Number:
4. Herd Number:
5. Dip Number:
6. What is the weight (kg)/size of the animals to be treated?
7. What Product was used last to treat this condition?
8. Have any of the animals to be treated had an adverse reaction?
9. Please acknowledge that you have read the appropriate Animal Medicine
Datasheets.
CONTINUE
Once this information has been implemented and they press continue, this
should then take them into the credit card page or further shopping.

Adding a link to a PDF datasheet is straightforward, but the only way I can think of making this work with only certain products required to have this is to have a link in the product description field which opens a 'pop-up' form to email page, which the user purchaser should fill in before the product could be despatched.
Anybody had a similar situation and how have you managed to solve it?
