Where is the root place that the 'overall page layout' is set? Everywhere I can see says ' Use Parent (....'

in Setting > Site Options > Layouts

Any that say 'use parent' here, detail the variable at the bottom which you can find in the Design > Library where it is set at instead.

In this settings page the overall page layout still says - use parent... whereas all the others have the actual layout listed. Where is the next level up for the overall page layout?

I did answer that in my initial reply.

I believe the main overall layout variable name is 'StandardLayout'
Look in your Design > Library > Layouts under "Web Page Outer Layout" and open up this selector called 'StandardLayout'

At the bottom of the 'Basic Definition' tab, you'll find it' initial value set.

A lot of the layouts are also set in the design settings menu which propagate down the site until over ridden locally.
