SagePay Simulator Form

Hi,

I am using the SagePay test Simulator PSP:
https://test.sagepay.com/Simulator/VSPFormGateway.asp

I have tried to simulate an order but each time I get these errors:
BillingSurname:
No BillingSurname was supplied.
BillingFirstnames:
No BillingFirstnames was supplied.
BillingAddress1:
No BillingAddress1 was supplied.
BillingCity:
No BillingCity was supplied.
BillingCountry:
No BillingCountry was supplied.
DeliverySurname:
No DeliverySurname was supplied.
DeliveryFirstnames:
No DeliveryFirstnames was supplied.
DeliveryAddress1:
No DeliveryAddress1 was supplied.
DeliveryCity:
No DeliveryCity was supplied.
DeliveryCountry:
No DeliveryCountry was supplied.

But I have filled in both delivery address and billing address on my test orders!

Any ideas of what I need to do to get it working?
