This is probably to do with the shopping mode that your site/section is using. For the 'Other Information' prompt and the 'Date Prompt' to show against the product online the shopping mode must be set to 'Quantity on Product Page'. For 'Quantity on Confirmation Page' and 'Quantity in Shopping Cart' these prompts will display when the customer adds the product to cart.
The default shopping mode is set in 'Design | Design Options', but this can be overridden at the section level on the 'Page Settings' panel of the section details.
