This problem has reared it's ugly head again.
Phase not found (Phase: 0, !D: 2464, Language: eng-uk)
Phase not found (Phase: 0, !D: 2665, Language: eng-uk)
Phase not found (Phase: 0, !D: 2451, Language: eng-uk)
Phase not found (Phase: 0, !D: 2452, Language: eng-uk)

I wonder has anyone spotted any glaring causes and come up with a simple solution by any chance?

I too have this exact same problem. Once I've clicked okay the 4 times actinic then opens.

I've only had this problem since upgrading to V11.0.1 LMFA earlier today.

I've raised a query with support who were unable to replicate it with my database. Got to send them a snapshot of the site now.
