That's even stranger then Jules, the fact that you manage to use it sometimes is really weird. If the layouts aren't in your design library, what does it show. Perhaps your design library is somehow showing the old catalog one. I think yours is a different issue to the one being fixed in this thread.

Have posted here as it is still a Moto form layout problem although the page is not blank everything is too close together and overlapping.
This is a clean install of 9.03 on a new Vista PC with an upgraded V8.5.0 site.
It would appear that all the text input boxes on the centre part of the Moto form are too far to the right and end up going underneath the left column and of course can not bee seen.
I have a wide screen monitor and the form is all squashed in 2/3 of the screen. Image below.
Any ideas?

I haven't seen this issue before but could you try reverting your 'Standard MOTO Form' back to factory settings please (Design | Library | Layouts | Moto Form group | right-click on the 'Standard MOTO Form' layout and select 'revert to factory settings).********************

So there we go. Sounds to me like an oversight - I can't fathom why Business doesn't allow users to edit the MOTO layouts, given that the MOTO form is available.

Hi Jason,
OK, I've found the problem and it's because you have set an image in the 'Keep Details Private' prompt (Design | Text | Phase 0, ID 15), which you are using for:
"I do not wish to receive the free Multisizer pictured below or receive promotions or discounts in the future".
This is pushing the next column across.
I suggest that you find the 'MOTO Keep Details Private' layout and replace the 'InvoicePrompt015' variable with your text above so it does not show the image.
I'm not sure why you can't revert that layout back to factory settings though as I was able to without problems. I'll ask development if they have any ideas about that.********************

I'm getting a blank MOTO form.
I have followed the instructions, when I click on the Page Layouts tab in Advanced Themes Configuration I get another infamous crash which produces a DMP record.
I've gone into the Library > Variables tab. I've checked the variable called 'MOTOShortcutButtonLabel' - both the Initial and Top Level values are blank. I get a question mark when I hover over the 'Values' tab. I get the same thing on 'MOTOShortcutProductReference'. I then backtracked to the 'Layouts' tab.
I'm kind of assuming and hoping this is the same as every other Selector in the library - the values go kaput and the only way to fix it is changing the layouts offered in the Values lists. (Moving them from the available to offered lists, then selecting an Initial and TL value).
In the Library 'Layouts' tab, under every Layout Type, I've checked the Values of the Selectors. They all seem to have layouts in the 'Available Layout Names' list, not the 'Layouts Offered By This Selector' list.
I'm hoping somebody will be kind enough to check their Layouts in the library and see the status of 'MOTO' related layout values are - presumably the Initial and TL values are not supposed to be empty.

I've taken a look at a snapshot that you sent in for another issue and can see that the IDs for your MOTO Form are a bit messed up. I've tried to put it right but am still getting a crash when on the New Order tab although you can now see the form, so I've put it to development to get it sorted out for you.********************
